PHOTO & MEDIA CONSENT AND RELEASE FORM

I. INDIVIDUAL CONSENT & RELEASE (For Participants, Guests, and Attendees)

By completing and submitting this form, I grant permission to Smoke Tree Stables and Horse Human Partnership (collectively, "STS/HHP") to photograph, film, or record me during my participation in any STS/HHP activities, events, or training sessions.

I understand and agree that these images and recordings may be used for, but not limited to:

• Advertising, marketing, and promotional materials

• Website, social media, and digital distribution

• Print publications, brochures, and media kits

• Educational, training, and commercial purposes

I understand that I will not receive compensation for the use of these images and that STS/HHP retains all rights to the media for indefinite use. I release STS/HHP and any affiliated entities from any claims related to the use of my image, including but not limited to rights of publicity, privacy, or defamation.

I acknowledge that participation is voluntary, and I waive any right to inspect or approve the final use of the images.

III. GENERAL TERMS & CONDITIONS

1. Ownership & Copyright:

   • STS/HHP maintains unrestricted rights to use any approved images. Photographers retain copyright but grant STS/HHP a non-exclusive, perpetual license for use.

   • Participants waive all claims to future compensation, royalties, or control over media usage.

2. Revocation & Withdrawal:

   • Participants may revoke consent by submitting a written request to info@smoketreestables.com. However, revocation does not apply to materials already published.

3. Liability Release:

   • STS/HHP is not liable for any third-party misuse of images once publicly released.

4. Governing Law:

   • This agreement is governed by the laws of the State of California. Any disputes will be resolved in Riverside County, CA.
